XEO HOUSING APPLICATION

The following information/documentation is required to process your application form:

  1. Birth Certificate, Valid Passport, or Naturalization Certificate for all household members.
  2. Social Security Card for all household members.
  3. Valid State-Issued Photo ID for all household members age 18 and older.
  4. All Sources of Income for the Current Year for all household members age 18 and older.
  5. i.e. Benefit/Verification letters for Social Security, Pension, Annuity, Alimony, Child Support, Public Assistance, Unemployment, and most Recent Bank Statement).
  6. Employer Information for all household members age 18 and older.
  1. Housing History for the past 5 years for all household members age 18 and older.
  2. Proof of Renter's Insurance.
  3. IF APPLICABLE: DD Form 214, Discharge Certificate, Official Letter, or Military ID Card.
  4. IF APPLICABLE: Documentation of Displacement by Appropriate Authority.
  5. IF APPLICABLE: Documentation of Domestic Violence by Appropriate Authority.
  6. IF APPLICABLE: Documentation of Full-Time Student Attendance by Appropriate College, University, or Technical School.

To avoid delays in processing or denial of your application, please

do not submit your application form until the above documentation is gathered.
