THE FOLLOWING INFORMATION/DOCUMENTATION IS REQUIRED:

1. Birth Certificate, Valid Passport, or Naturalization Certificate
2. Social Security Card
3. Valid State-Issued Photo ID
4. Documentation of all sources of income for the current year
5. Employer Information
6. Documentation of housing history from the past 5 years
7. Proof of renter's insurance
8. IF APPLICABLE: Documentation of military, domestic violence, or student status.

Please contact our main office at 724-888-1133 once the above documentation is gathered. We will provide you with an application form.

4. Spayed/neutered status
5. Vaccination records from veterinarian
6. Description of temperament and behavior
7. Proof of renter's insurance
8. IF APPLICABLE: Documentation for service animal or emotional support animal
